Register for Okoboji Youth Choir

Okoboji Youth Choir is a community choir for area students ages 2nd grade-8th grade.

We run on a 10-week session in the Fall and in the Winter, with a short performance at the end of each term. The membership fee of $100 will cover the ENTIRE 10 weeks and must be paid prior to the first rehearsal.

This fee covers sheet music costs, venue rental, and director & accompanist fees. If you are in need of financial assistance, please let me know that in the form below.

Are there any allergies or health issues that we need to be aware of for you?

Sometimes we use images or videos of our students for publicity or other promotional materials for Okoboji Youth Choir. Please choose from the following options:

Rehearsals will be an hour long. Time and place will be determined a few weeks before rehearsals begin. If you had a preference, would you choose a Tuesday afternoon/evening, Thursday afternoon/evening, or Sunday afternoon/evening?

By signing my name in the box below, I agree to have my child dropped off for rehearsal no more than 10 minutes early and picked up after rehearsal no more than 10 minutes after rehearsal end time. I understand that Jessica and the rehearsal venue have other commitments and I will adhere to the choir schedule that will be sent out prior to each term.

I understand that neither Jessica Schable, the rehearsal venue, nor the Okoboji Youth Choir organization will be held responsible for any injuries, accidents, or consequences resulting from inappropriate behavior, failure to follow instructions, or horseplay during rehearsals, classes, or performances.
